What is CHADIS?

CHADIS is an online system of standardized questionnaires that help Dr. Kim review the development and mental health of your child. These questionnaires tell Dr. Kim what happens in your child's day-to-day life so she can give your child the best possible care.

How to Register for CHADIS

  1. Visit site.chadis.com and click Register (or go directly to the registration page here).
  2. If you have never used CHADIS, select "No, I have never used CHADIS before". If you already have a CHADIS account from another doctor (or from previous visits), select "Yes, I have used CHADIS before".
  3. Add the invitation code "9033423710" and fill out the requested information. For this step, use your information. You will register your child in the next step.
  4. Fill out the information for your child.

How to Add a New Patient to CHADIS

  1. Visit site.chadis.com and click Login (or go directly to the login page here).
  2. Log in to CHADIS.
  3. Click on "Add another child/patient". (If you have older children who visit other doctors, select "Enter an invitation code you have received for another child/patient".)

How to Invite my Teen to CHADIS

Teens will have their own set of questionnaires starting at age 11. You can add them through your account while you are doing CHADIS before your visit, or they can complete their questionnaires before their appointment on the tablet.
  1. Visit site.chadis.com and click Login (or go directly to the login page here).
  2. Log in to CHADIS.
  3. Click on your child's name.
  4. Click the red "Invite (Teen) to do CHADIS" link on the questionnaires page.

How to Invite a Teacher to CHADIS

Dr. Kim may ask you to invite a teacher to CHADIS to get more information about their learning or behavior. Teachers do not have access to your profile and are only prompted to answer teacher questionnaires. All information will go directly to Dr. Kim so she can provide the best possible care.
  1. Visit site.chadis.com and click Login (or go directly to the login page here).
  2. Log in to CHADIS.
  3. Click on your child's name.
  4. Click "Invite another Person to do CHADIS" on the questionnaires page.
